Functional & Integrative Medicine Assessment

Time to see how the pieces fit.

A longer first appointment for concerns that span more than one part of you: sleep, energy, gut, mood, hormones, pain. We bring together your history, your results and the life around them. You leave with a prioritised plan, and a team that follows it with you.

Who comes to us

You might recognise yourself here.

None of these is a diagnosis, and you don’t need to fit one to book.

What’s included

Before, during and after.

Before you arrive

  • A secure intake form: your history, current medicines and what you want to change.
  • Bring what you have: recent results, letters, imaging, and a list of your medicines and supplements. You don’t need new tests to prepare. If you don’t have your records, we can request them from your GP with your consent.

In the appointment

  • A detailed review of your health history, symptom timeline and previous investigations.
  • Your current treatment, nutrition, sleep, stress and daily function, looked at together.
  • Discussion of investigations or referrals, only where they could change your care. Any test is explained and priced before it’s ordered.

After

  • A prioritised plan for next steps and review, written down and kept in your shared Sohma record.
  • Follow-up appointments ($89, or $149 extended), and nurse check-ins between them at no charge.

60 or 90 minutes?

Choose 90 minutes if there’s a lot of history, or several things you want to prioritise. The 90-minute assessment is with a nurse practitioner.
