How switching works

Four steps.

  1. Book Transfer of Care.

    A 30-minute appointment, $129, to review your treatment and records independently. If you need a broader assessment, reception can help you choose.

  2. Bring what you have.

    Recent results, your current medicines and treatments, and any letters from other clinicians. They help, but they’re not required: we can request your records from your previous provider with your consent.

  3. We look at where you are now.

    Not just where your health was. The first conversation starts with your story, not with paperwork.

  4. We build a plan together.

    You leave with a written plan and clear next steps, kept on one shared record, with follow-up and nurse check-ins once you’re with us.
